Duitsland heeft informed consent voor de Mirena ingevoerd

In Duitsland heeft men vorig jaar het zogenaamde 'informed consent' ingevoerd. Vrouwen die een Mirena laten plaatsen, worden van te voren ingelicht over het risico op baarmoederperforatie en de verdenking van een hoger risico op borstkanker.

Duitsland heeft voor Mirena (een spiraal als anticonceptie) ‘informed consent’ ingevoerd, vrouwen die Mi-rena krijgen toegediend moeten van tevoren verklaren dat ze op de hoogte zijn van alle waarschuwingen. Het gaat dan vooral om het mogelijke risico op uterusperforaties, maar ook dat ze zich bewust zijn van een contraïndicatie ([verdenking op] borstkanker). Ook wordt extra gewaarschuwd voor het risico op ecto-pische zwangerschappen. Het Lareb heeft enkele meldingen ontvangen van uterusperforatie, het zijn ech-ter geen grote aantallen.

Dat het Lareb geen grote aantallen perforaties te melden heeft komt omdat veel gevallen daar gewoonweg niet worden gemeld:

Twice as many uterine perforations with Schering's Mirena than with copper IUDs
PARIS, March 30 (APM) - The incidence of uterine perforation in women using Schering's levonorgestrel-releasing intrauterine system (LNG IUS) Mirena seems at least twice as high as that with copper IUDs, according to a Belgian and Dutch study.

Uterine perforation is a potentially serious complication due to the insertion of an intrauterine device (IUD). The risk of uterine perforation is estimated at between 0 and 1.3 per 1,000 insertions, excluding LNG IUSs, say K Van Houdenhoven from the AZ Sint Maarten hospital in Mechelen, Belgium, and colleagues.

A recent report of 14 cases of extrauterine IUDs showed that nine of these were LNG IUSs, suggesting that the latter have a higher rate of localisation errors.

In order to estimate the incidence of uterine perforation linked to Mirena insertion and to identify possible risk factors, researchers asked gynaecologists from hospitals in the province of Limburg in the Netherlands about uterine perforations possibly linked to LNG IUSs between 1999 and 2002.

A total of 21 cases were reported by the gynaecologists, who had all performed at least 10 insertions of LNG IUSs per year.

On the basis of figures concerning sales of Mirena in this region in 2000, 2001 and 2002, the researchers estimated the incidence of uterine perforation related to Mirena insertion at 2.6 per 1,000 insertions.

They noted than none of the 21 perforations were reported to the Dutch pharmacovigilance centre Lareb, whereas this centre had recorded at least 24 Mirena-linked cases of uterine perforation in the Netherlands since the launch of the LNG IUS in February 1996. "This indicates a considerable underreporting of this complication," they said.
They also noted that the insertion of IUDs in lactating mothers, even more than six weeks after birth, is an important risk factor in uterine perforation.

The study shows that only in four cases did patients report that the insertion of the LNG IUS was very painful. Uterine perforation can thus easily go undetected at the time of insertion, in particular in lactating women who generally feel less pain on the insertion of an IUD, the researchers said.

They highlight the importance of following the existing recommendation of a routine check-up six weeks after the insertion of Mirena with a transvaginal scanner in order to determine the location of the IUD. They also propose that a scan be carried out immediately after insertion, especially if this was difficult or painful for the patient.

Finally, they emphasise that it is highly desirable to report adverse effects not only of drugs but also of IUDs to national pharmacovigilance centres.

Onderaan deze pagina staat een uittreksel van het onderzoek dat wordt genoemd. Helaas heb ik het wel in PDF kunnen vinden, maar dan moet je er voor betalen (wat ik belachelijk vind ;) )
